What Look Do You Want?

All women considering having a Breast Augmentation are primarily concerned with exactly how their breasts will look afterwards.

If this applies to you, you are likely asking the following questions with regard to how your ideal breasts might look:

  1. How big should my breasts be?
  2. Where on my chest should my breasts sit?
  3. What shape should my breasts have?
  4. What should my cleavage look like?
  5. How pert should my breasts be?

 

While most women would imagine that everybody wants to look natural, the fact is that more and more women are requesting the Fake Look Breast Augmentation. Whether you want a fake look or a natural look result is entirely up to you; it is, however, very important to clarify what characteristics distinguish the two looks.

What Look Is Best for Me?

This is a very common question patients ask Dr. Jugenburg.  However, ONLY YOU can answer this question.  You are the one that has to be happy every time you look in a mirror.  So what Dr. Jugenburg or anyone else likes doesn’t really matter.  You are doing this surgery to make yourself happy, not someone else.

To best determine what would make you happy, we recommend that you do research online and find at least 10 photos of breasts that you love, and 10 photos of breasts that you hate.

But here is the catch:  

You must look for nude photos.  Women wearing clothes, bikinis or bras do not show their true breasts.  They always show a breast that is lifted, supported, held in place where it wouldn’t normally sit were that person naked.  These are not ‘realistic’ photos.

For that reason it is important that you find nude photos.  Ideally viewed head on, not from an angle, not on a person lying down or some other pose. Head on, standing, showing full breasts.  Looking at our or other surgeon’s before and after photos is your best bet to find photos that you like.  And don’t like.

These photos will help you narrow down what you do like and what you do not like.  These photos will also help Dr. Jugenburg better understand what you want to achieve.

Natural Look Breast Augmentation

Women always analyze one another, assessing and comparing clothing, body shape, image and behavior. The decision to have Breast Augmentation is, however, extremely personal. One of the main advantages of a natural looking breast enhancement is that few other people will necessarily know that you have had an augmentation unless you wish for them to do so (this is particularly true if you have previously worn a padded bra and/or bikini). Your scars will be beautifully concealed, virtually undetectable to the naked eye, and your breast enlargement will compliment and enhance your figure.

The characteristics of a typical natural looking breast enlargement are:

  1. The breast has a gentle slope to it and projects forward towards the nipple
  2. Most of the volume is in the lower half of the breast
  3. The nipple sits on the most projecting part of the breast
  4. The breast sits approximately at the mid level of the arm
  5. The nipple is located above the breast fold (if the nipple sits below the breast fold, that by definition is a drooping breast)

How to achieve Natural Looking Breast Augmentation?

In order to achieve ‘The Natural Look’, the breast augmentation surgery has to be carefully planned and executed.  It is important that the augmentation simply enhances your existing breast, and not tries to build a new one.

To achieve a natural looking results, some or all of these have to be considered:

  1. Placing the implant under the chest muscle (sub-muscular or sub-pectoral)
  2. Using a moderate profile implant that fits your chest width and is proportional to the rest of your body. Your breasts will initially sit higher on the chest immediately after Breast Augmentation surgery, but with routine and deliberate manipulation of the implants (through breast massage) they will quickly descend into a more natural position within the first 3 months.
  3. Limiting the breast implant width to the width of your breasts.  If you chose an implant that is larger, it’s edges will peek out from around the natural breast borders.
  4. Using a teardrop shape implant in patients who have no breast shape and need the implant to shape the final result.

Fake Look Breast Augmentation

The Fake Look is inspired by Hollywood, where celebrities with very obvious breast enlargements are often seen. Despite not being a natural look, the extensive media exposure of this type of breast enhancement has led to it becoming accepted by some women as normal (or a normal), thereby further confusing the distinction between what is considered fake and what is considered normal.

The characteristics of a fake-looking breast enlargement are:

  1. Breasts that are positioned high on the chest
  2. A prominent, round or bubbly cleavage (giving a permanent push-up bra effect)
  3. A breast that appears large in proportion to the rest of the chest
  4. Contouring around the edge of the implant can be seen
  5. Rippling or other features of an excessively large breast are seen

The Fake Look is achieved by going over the chest muscle (sub-glandular or sub-mammary), using high profile implants or filling saline implants to their maximum, and using an implant size that is large compared to your chest. Immediately after Breast Augmentation surgery, your breasts will more closely resemble the final result than with the Natural Look augmentation (particularly once any swelling has gone down).

Breast Augmentation Surgery

All Breast Augmentation procedures initially involve the careful placement of a small incision through which a breast pocket is then created. At the Toronto Cosmetic Surgery Institute the incision can be made in one of three possible places on a woman’s body:

  1. Breast fold (Infra-Mammary Breast Augmentation)
  2. Areola (Peri-Areolar Breast Augmentation)
  3. Armpit (Trans-Axillary Breast Augmentation)

The breast pocket is an empty space either above or below the chest muscle (pectoral is major) into which the breast implant is inserted.The position of the implant above or below the chest muscle will influence the final appearance of the breast enhancement.

Your Breast Implant Placement

Breast implants that are positioned below the chest muscle tend to look more natural because they are hidden by both muscle and breast tissue. Placing the implants over the chest muscle and under the breast itself (sub-glandular) gives a rounder, fake-looking breast enlargement.

If this placement is to be chosen, it is important that you have sufficient upper breast tissue for proper implant coverage, otherwise a prominent upper edge to one or both implants may be seen.

The benefits of an implant placement below the chest muscle (sub-muscular or sub-pectoral):

  1. More natural look
  2. Less visible implant in the upper breast
  3. Lower risk of Capsular Contracture (undesirable implant hardening)
  4. Lower risk of implant bottoming out (unfavorable downward movement)
  5. Less visible implant rippling/wrinkling (natural folds in the shell of an implant)
  6. Mammograms are easier to read (the implant is not directly behind the breast tissue)
  7. Surgery can be performed through a small incision either hidden in the armpit or concealed in the areolar border leaving a virtually undetectable scar (for further information on Scarless Breast Augmentation, please refer here)

The benefits of an implant placement above the chest muscle(sub-glandular or sub-mammary):

  1. More bubbly, rounded (or push-up) look
  2. Less discomfort during post-surgical recovery (this is minimal, patient dependent, and should not be used as a determining factor)
  3. Quicker post-surgical recovery
  4. Final (optimal) size and shape seen earlier
  5. Surgery can be performed through a small incision beautifully concealed in the areolar border leaving a virtually undetectable scar

For many women, placing the implant below the chest muscle and partially behind the breast tissue overlying the muscle (Dual Plane Breast Augmentation), while also altering some of the muscle, will achieve optimal improvements to the breasts and allow for a subtle lift. It is also possible to achieve a fake-looking breast enlargement using this procedure, but this will depend on the extent of alteration of the muscle and the size of implants being used.

Which implant placement is best for your Breast Augmentation? This will be determined by how much your breasts are drooping, how much glandular breast tissue you have, whether or not your breast tissue needs to be repositioned relative to the chest muscle, and what look you want to achieve. During the consultation process, Dr. Jugenburg will examine your existing breast anatomy and explain to you which implant placement will give you the best results.

Your Breast Implant Profile

The profile of a breast implant refers to the distance an implant will project forward from your chest wall. A low profile implant is very flat, while a higher profile implant will protrude forward (stick out) from your body and create a more prominent breast. Typically, the higher the profile on any given woman, the more fake-looking the breast enlargement.

When choosing your breast implant profile, your existing breast width is a key factor. In order to achieve a proportionate appearance, it is important to ensure that the implant diameter (or base width) is equal to or marginally smaller than your existing breast width. Women with very little existing breast tissue and a narrow chest wall (described as petite), are therefore often limited to implants that are much smaller than they had imagined their ideal size to be. In such situations, keeping the implant diameter small, but increasing the profile of the implant (i.e., the extent to which the implant protrudes forward), permits the use of a larger size implant without it being too wide for the patient’s frame.

Your Breast Implant Size

The size of your breast implants will make a big difference to the look you can achieve. As with the breast implant diameter mentioned above, the primary goal of most women is to achieve a proportionate appearance. During the consultation process, Dr. Jugenburg will measure your chest and breast size, as well as your height and weight. These measurements will be used to recommend the right implant to fit your existing proportions, a method known as Dimensional Breast Augmentation. While you might instead decide to choose a size that goes beyond what has been suggested if you desire a more fake-looking enhancement, this is your decision. However, Dr. Jugenburg will always guide and advise you on what will be the most beneficial choice for your body and needs.

Breast implant size is measured based on volume in cubic centimeters (often referred to as cc’s), not in traditional bra cup sizes; the larger the implant, the greater both the volume of cc’s and the projection from the chest wall (for a given implant diameter). By consulting the breast implant volume sizing charts that will be shown to you by Dr. Jugenburg and trying on various sizes (or samples), you will have in-person visualization of how different implant sizes will affect your shape. When making your decision it is important to consider that larger implant sizes may be problematic for women having a more active lifestyle (e.g., participating in sports, jogging and yoga) and are associated with a greater risk of complications, such as bottoming out (an unfavorable downward movement of the implant).
